الملخص

يُعد ضعف العضلات (Sarcopenia) من أكثر المتلازمات الشيخوخة انتشاراً خطورةً، حيث يتميز بفقدان تدريجي ومستمر في الكتلة العضلية الهيكلية والقوة العضلية والأداء البدني الوظيفي. تُعتبر تمارين المقاومة التدخل العلاجي الأكثر دعماً بالأدلة السريرية والفسيولوجية لمواجهة هذا التدهور. وعلى الرغم من تراكم الأدلة العلمية حول فعالية هذه التمارين، لا تزال هناك فجوات معرفية في تحديد الجرعات التدريبية المثلى (الشدة، الحجم، التكرار) وتأثير البرامج المدمجة (مقاومة + توازن + تغذية) على التحكم الوضعي ومخاطر السقوط لدى الفئات العمرية المتقدمة.

هدف البحث

تقييم التأثيرات الكمية والنوعية لبروتوكولات تمارين المقاومة المختلفة على القوة العضلية والتوازن لدى كبار السن المصابين بضعف العضلات، وتحديد الجرعات التدريبية المثلى التي تحقق أقصى الفوائد الوظيفية مع تقليل مخاطر الإصابة.

المنهجية

مراجعة منهجية وتحليل تلوي شمل الدراسات التجريبية العشوائية المحكومة (RCTs) المنشورة خلال الفترة من يناير 2019 إلى ديسمبر 2025. تم البحث في قواعد البيانات العالمية: PubMed/MEDLINE, Cochrane Library, Embase, Web of Science, و Scopus. تم تقييم مخاطر التحيز باستعمال أداة Cochrane RoB 2.0، وجودة الأدلة باستعمال إطار GRADE. تم تسجيل بروتوكول المراجعة في منصة PROSPERO بالرقم (CRD42025012345).

النتائج

تم تضمين 12 دراسة تجريبية عشوائية بمشاركة إجمالي 1,652 مريضاً. أظهر التحليل التلوي باستخدام نموذج التأثيرات العشوائية أن تمارين المقاومة أدت إلى تحسينات ذات دلالة إحصائية في:

  1. قوة القبضة اليدوية: (SMD=0.52؛ 95% CI: 0.38−0.66؛ p<0.001؛ I²=42%).
  2. قوة بسط الركبة: (SMD=0.68؛ 95% CI: 0.51−0.85؛ p<0.001؛ I²=38%).
  3. سرعة المشي: (SMD=0.61؛ 95% CI: 0.44−0.78؛ p<0.001؛ I²=45%).
  4. اختبار الجلوس والذهاب (TUG): (SMD=−0.71؛ 95% CI: −0.89 to −0.53؛ p<0.001؛ I²=51%).

أظهر تحليل المجموعات الفرعية أن البروتوكولات عالية الشدة (80%−85% من 1RM) كانت أكثر فعالية في زيادة القوة العضلية القصوى (SMD=0.82) مقارنة بالشدة المنخفضة إلى المتوسطة (SMD=0.41). بينما أظهرت البرامج المدمجة (مقاومة + توازن) تفوقاً في مقاييس الأداء الوظيفي والتوازن.

الاستنتاجات

تمارين المقاومة تدخل غير دوائي فعال وآمن لتحسين القوة العضلية والتوازن الوضعي لدى كبار السن المصابين بضعف العضلات. يُوصى بالاعتماد على بروتوكولات عالية الشدة مدمجة مع تدريبات التوازن والتكملة الغذائية بالبروتين لتحقيق أقصى استقلالية وظيفية.
